Appointment

You can make appointment with our doctor by calling to our hot line. Our appointment staff will provide you with appointment date and time as well as provide you with other information for the service you require.

Form & Registration

All new patients need to register upon entering clinic. To expedite this process, you are encourage to complet the registration form in advance and guide to meet our doctor.

Consultation

Your physician consultation is very important, because it is the moment when you can discuss your medical issues face-to-face with your doctor. you will have a lot of questions to ask, so preparation is the key to make the most of your consultation. We would like to suggest you that:

- Come with your medical history.

- Bring any photos or visuals aids that will help explain what it is you want or expect from treatment.

Pre-Surgical Test

Prior to admission and surgery, your physician will require you to undergo laboratory investigations and/or dianostic examination to ensure that you are fit to surgery. These tests are clinic standard procedure and they do incur a cost. Some package include pre-surgical tests and dianostics as part of the total package price, while other do not. Please consult with your physician.
